Contact form sometimes doesn't end up as a topic on the forums

I just realised one of the members filled in the Contact form (Using FF) but it didn't end up as a thread in a specified forum as intended. I just checked in the logs by chance. If there's no fix for this, can I somehow export it manually as a thread or jump start it by activating a task or something? This is very important to me.

Topics are only be triggered by the send function and can't be created after. Can you open a ticket with version number and ipb version. When it comes to topics not posting, it can be caused by many different things.
To be honest, I can't reproduce this myself. I tried over and over and over again, submitting the form in FF or IE and it always shows up as a thread. Recently, one of my members told me submitted the form and I saw it in the logs, meanining, he did everything right. I asked him to try to resubmit the same form in the same browser and this time it worked.

He was logged in at the time, not guest? You have the same forum selected for all subjects?
I didn't allow guests to use the system so he was logged in. I have only one subject set (The Recruitement Form, as I run a gaming clan) and also only one, (public) forum where it's supposed to end up.
I asked him after he made the submission for the first time, if he received the confirmation screen, he said yes.
I have LOTS of member groups and 80% of them are allowed to use the system. Since it's a weird one, I'll let you know if it happens again.

It happened again today...happens seldomly but it does happen..no idea what's causing it. I get the PM but there's no thread generated in the forum.
